Thank you for providing comments on the “Procedures for Processing the Engineering Service Provider Infraction Report”. The responses to the comments are attached. As a result of consultation, the following two documents have been revised. The revised documents are attached.
• Procedures for Processing the Engineering Service Provider Infraction Report
• Form PH-EC-900, Engineering Service Provider Infraction Report
The remaining four documents are not subject to any revisions. The updated documents (total six as per the original posting) will be published on ministry’s Technical Publications portal.
Effects of consultation

The MTO has updated the procedures for processing Infraction Reports/Warning of Infraction Reports for Contractors and Engineering Service providers, including related forms. The following documents have been updated and attached to this notice:
o Procedures for Processing the Engineering Service Provider Infraction Report
o Procedures for Processing the Contractor Infraction Report
o Form PH-EC-900: Engineering Service Provider’s Infraction Report
o Form PH-EC-901: Engineering Service Provider’s Warning of Infraction Report
o Form PH-CC-832: Contractor’s Infraction Report
o Form PH-CC-833: Contractor’s Warning of Infraction Report
The changes are mainly editorial and include:
o Instructions to upload the reports onto Contract Management System (CMS)
o Secretary’s generic email (qualificationcommittee@ontario.ca)
o Updates related to MTO Transformation
o Reference to Technical Publications site as the repository of the Procedures and Forms
o Deleted duplicate wording regarding roles/responsibilities of the Qualification Committee and referenced the Qualification Committee Procedures
o Deleted delivery address. Deliveries shall be via CMS or email.
